Order Forms
When ordering materials, purchasing tickets or making a
donation online, our site's order forms require users to give us
contact information (like your name, e-mail address and mailing
address) and financial information (like your credit card number and
expiration date). We use this contact information to
process, fulfill and acknowledge your order or donation and to send you information about sarcoidosis and about FSR.
This site has security measures in place to protect the loss, misuse and
