From AUGUST 30 – SEPTEMBER 1, 2020, FSR is hosting its patient education summit in Chicago, IL. This will be a two-day event where patients can hear from experts in the sarcoidosis field as well as network with other patients and caregivers.

To register for the event on August 30 – September 1, please click “Buy Tickets” below. If you previously registered for the Chicago Summit when it was set to take place in April, your registration has been automatically applied to the new conference date. If you’re unable to attend the new event date and would like to either be reimbursed or transfer your registration to a different FSR Summit, please email Summits2020@stopsarcoidosis.org

 

FSR Sarcoidosis Patient Education Summits are a valuable educational resource for those living with sarcoidosis and their caregivers.

About this Event

FSR’s weekend-long summits will provide presentations from expert physicians and researchers, as well as lectures and exercises in lifestyle, emotional well-being, legal and financial matters, and FSR resources. Attendees will be able to choose which presentations to attend based on which are the most relevant to their lives. There will also be a self-care station and ample opportunities for attendees to relax and recharge between sessions.

If you have already made a hotel reservation for the previous summit dates (April 2020) your reservation has been automatically canceled. You will need to book a new reservation using the new group code for the new dates.

Scholarship Information

FSR is pleased to make our summit more accessible to all by offering a limited number of travel, accommodation, and registration grants for each summit. Please read all of the information below and contact summits2020@stopsarcoidosis.org with any additional questions.

Thank you for your interest in applying for a need-based registration waiver for the Chicago Patient Education Summit. Please read carefully before applying for a registration fee waiver.

  • Registration fee waiver is meant to cover the cost of a summit registration fee for one person. This includes all features of summit registration including breakfasts and lunches on Saturday (4/4) and Sunday (4/5), access to the Wellness Center, and all sessions.
  • If you and a caregiver wish to attend together, you must include their name and information as well as provide documentation of shared residence at time of application. Applications without caregivers listed will not have the option to request a caregiver registration fee at a later time. Your caregiver would need to submit their own, separate application.
  • Registration fee waiver does not cover travel, or hotel accommodations.
  • The Registration Fee Waiver will not exceed the value of summit registration ($60/per person).

Eligibility Requirements:

The eligibility requirements for a needs-based registration fee waiver are as follows:

  • Demonstrated financial need based on household income and the ability to provide documentation.
  • Confirmed diagnosis of sarcoidosis
  • FSR reserves the right to decline a registration fee waiver application should an individual provide misinformation on his/her application or otherwise show misconduct at any FSR event.
OPEN NOW: Travel and Accommodation Grants – these applications will close on January 27, 2020. You may apply for a registration fee waiver prior to applying for a travel or accommodation grant.

Apply for a Travel and/or Accommodation Grant

Travel Grant:

Thank you for your interest in applying for a needs-based travel grant for the Chicago Patient Education Summit. Please read carefully before applying for a travel grant.

  • Travel grants are needs-based and distributed based on the eligibility requirements outlined below. We will allocate grants on a first-come, first-serve basis. Once all travel grant funds are depleted, the applications will be closed.
  • Travel grants are meant to offset your costs, and may not cover all of your expenses. We strongly encourage you to research transportation costs early to better understand the additional amounts for which you may be personally responsible.
  • You do not need to be registered for the Chicago Summit at the time of applying for a travel grant. However, applicants who did not also apply for a registration fee waiver, must register for the Chicago Summit within seven (7) days of travel grant award email, unless other arrangements with FSR have been made in writing.

To apply for a registration fee waiver only (no travel expenses covered), please review the Registration Fee Waiver section below.

Eligibility Requirements:

The eligibility requirements for a needs-based travel grant are as follows:

  • Demonstrated financial need based on household income and the ability to provide documentation.
  • Confirmed diagnosis of sarcoidosis
  • Priority is given to people with advanced sarcoidosis, with additional funds available for people with sarcoidosis at any stage and caregivers of people with advanced sarcoidosis.
  • FSR reserves the right to decline a travel grant application should an individual provide misinformation on his/her application or otherwise show misconduct at any FSR event.

Travel Grant Allowances and Rules:

  • Travel grants will cover the cost of coach airfare only.
  • All travel must be booked through FSR’s Travel Agency, Travelex. FSR will not reimburse for travel costs.
  • Travel grants do not cover baggage fees, upgraded seats, meals, etc.
  • Travel grants do not cover hotel accommodations, or summit registration fees.

 

Accommodation Grant

Thank you for your interest in applying for a need-based accommodation grant for the Chicago Patient Education Summit. Please read carefully before applying for an accommodation grant.

  • Accommodation grants are needs-based on the eligibility requirements outlined below. We will allocate grants on first-come, first-serve basis. Once all accommodation grant funds are depleted, the applications will be closed.
  • Accommodation grants are meant to cover up to two-nights double occupancy hotel accommodation at the Chicago DoubleTree Magnificent Mile only. Parking fees may be covered when arranged in advance.

Eligibility Requirements:

The eligibility requirements for a needs-based accommodations grant are as follows:

  • Demonstrated financial need based on household income and the ability to provide documentation.
  • Confirmed diagnosis of sarcoidosis
  • Priority is given to people with advanced sarcoidosis and their caregivers, with additional funds available for people with sarcoidosis at any stage and their caregivers.
  • FSR reserves the right to decline an accommodation grant application should an individual provide misinformation on his/her application or otherwise show misconduct at any FSR event.

Accommodation Grant Allowances and Rules:

  • Accommodation grants will cover the cost of up to two-nights (April 3 and 4, 2020), double occupancy hotel accommodation at the Chicago DoubleTree Magnificent Mile only.
  • Accommodations may cover parking fees, paid directly to the hotel as part of the room accommodation
  • Travel grants do not cover min-bar expenses, purchased movies, room service, food or drink, or any other hotel services.
  • FSR is not responsible for any damages to hotel furniture, equipment, or items lost during your stay.
  • Accommodation grants do not cover travel, or summit registration fees.

 

TERMS AND CONDITIONS

It is important that you read through all of the terms and conditions below. Failure to comply could make you ineligible for scholarship assistance from FSR.

  1. If you receive any or all of our summit scholarship grants and decide not to attend or would like to decline the assistance, please contact summits2020@stopsarcoidosis.org Failure to do so may make you ineligible for future scholarship support from FSR.
  2. If you did not receive a summit registration waiver, and you do not register for the summit within seven (7) days of receiving your grant award email, your grant will be cancelled.
  3. All travel grants must be booked through FSR’s official travel agency, Travelex. Information on how to do so will be provided at time of award email. FSR will not reimburse travel expenses.
  4. If you receive a travel grant, but did not apply for, or did not receive an accommodation grant, you are responsible for the cost of your own accommodations.
  5. If you receive an accommodation grant, but did not apply for, or did not receive a travel grant, you are responsible for the cost of your own travel to and from the summit.
  6. You must be at least 18 years old to receive a travel or accommodation grant.
  7. Should your transportation reservations be or need to cancelled, FSR is not responsible for any related fees.
  8. If you do not attend the summit, and did not notify summits2020@stopsarcoidosis.org in advance, you will not be eligible for future scholarship support with FSR.
  9. You must attend all of the conference sessions. Failure to do so may make you ineligible for future scholarship support.
